Panel Discussion: The Digital Primary & Secondary Care Interface

The session will discuss care-coordination, the digital interface and collaboration between care settings, visibility of activity, a system-wide approach to referral management, advice and guidance, and the role of digital to ensure patients are directed to the right place, at the right time.
